Ellipsis Health, the AI care management company trusted by seven of the top 20 health plans in the U.S. and leading academic medical centers and integrated delivery networks, today announced its acquisition of Kouper Health, an AI care navigation company that builds deeply integrated agents for workflows across health systems and provider groups.

The acquisition brings together Ellipsis’s emotionally intelligent AI Care Manager, Sage, with Kouper’s native EHR integrations and longitudinal patient maps. Together, the companies aim to move healthcare beyond episodic interactions toward continuous, intelligent support for patients across the care journey.

Health systems are adopting AI at a rapid pace: 75% have adopted at least one AI solution, up from 59% the year prior, yet clinical capacity is limited. Patients move between appointments, assessments, treatment, and discharge, but the coordination and follow-up needed to support them often fall through the gaps.

AI creates an opportunity to change this, but its impact depends on more than intelligence alone. It requires technology that works reliably inside clinical workflows, better understands patients’ individual contexts, and follows through on the tasks that connect one stage of care to the next.

Founded in 2022, Kouper has raised funding from investors including CVS Health Ventures, General Catalyst, and 25madison. Its technology embeds AI agents into health system workflows and builds longitudinal patient maps that bring information from across a patient’s care into a more unified view. With Kouper’s integrations, Sage will gain the patient context it needs to match people with the right providers and schedule appointments directly in the EHR.
